Create a new opening and fill out the necessary information.
Make sure under 'How do you want people to apply?', that 'Applicants submit a custom form with Recruiting+' is selected.
Select Next to proceed to the Custom Form.
Next select how you would like to create your form. If this is your first time, you'll select Start From Scratch > Blank template.
An Edit Form page will open where you can edit your title and add a description. Then you can add a section, to start adding to your custom form. Select + Section.
You can add a Section Title and Description, and then click + Question to add your first question to your section.

Then select the question type that you'd like to ask - there are 10 types:
Text: The applicant enters this field with a few words (used for small answers)
Text Area: The applicant enters this field with many words or a paragraph (used for long answers)
Yes or No: The applicant selects either Yes (blue toggle) or No (gray toggle)
Number: The applicant enters a number
Date: The applicant chooses a specific date
Multiple Choice (single selection): The applicant can select 1 option from a list.
Multiple Choice (multiple selection): The applicant can select multiple options from a list.
Signature: The applicant enters a signature.
Phone: The applicant enters a phone number.
Upload: The applicant can upload a file.
Next, you can toggle whether the question is optional or not (blue = yes, gray = no).
Once your question has been added, you can choose to keep adding questions to the section, create a new section or Save your progress. You can keep building out your form until all fields are created.
